What is Alli (Orlistat)?

Alli (orlistat 60mg) is an MHRA-approved weight loss capsule manufactured by Haleon (formerly GSK). It is a pancreatic lipase inhibitor — a medicine that works directly inside the gut to reduce the amount of dietary fat your body absorbs from food.  


Unlike appetite suppressants or stimulants, Alli does not act on the brain or central nervous system. It works locally in the digestive tract, making it one of the most mechanically straightforward weight loss treatments available without a prescription in the UK. 


Alli belongs to the same drug class as prescription Xenical (orlistat 120mg) but is available over the counter because the 60mg dose carries a well-characterised safety profile suitable for self-directed use under pharmacist guidance.  

Both Alli and alternative Orlos contain the same active ingredient at the same dose; the key differences between them are manufacturer and retail price rather than clinical effect.

    Yes — in clinical studies, more than 40% of people taking Alli alongside a calorie-controlled, low-fat diet lost at least 5% of their body weight [1, 2]. 


    Alli is not a magic solution, and results depend heavily on dietary compliance, but it is a clinically validated aid for weight management in adults with a BMI of 28 or above. 

  • Alli begins blocking fat absorption from the first capsule. Early weight loss (within the first month) is typically modest — most people notice more meaningful progress over 3–6 months as the consistent calorie deficit from reduced fat absorption accumulates. Continued use alongside a low-fat diet gives the best long-term results.

  • Yes. Alli (orlistat 60mg) is available over the counter in the UK without a prescription for adults with a BMI of 28 or above. At Bliss, your clinician will carry out an eligibility check before your order is processed to ensure Alli is appropriate and safe for you.

  • Alli contains orlistat 60mg and is available over the counter for adults with a BMI of 28+. Xenical contains orlistat 120mg — double the OTC dose — and is prescription-only, generally prescribed for adults with a BMI of 30 or above (or 28+ with weight-related health conditions). 


    Xenical blocks approximately 30% of dietary fat compared to approximately 25% with Alli. Both products work via the same mechanism; the prescription dose is stronger and requires clinician oversight. 

  • Alli and Orlos both contain orlistat 60mg and work identically. The difference is brand and manufacturer: Alli is made by Haleon and is typically priced at a premium, while Orlos is made by Ethical Naturals Ltd and is usually available at a lower cost. If budget is a consideration and you are comfortable with a generic brand, Orlos and Alli are clinically interchangeable.

  • Orlistat has been used clinically since 1998 and has a well-characterised long-term safety profile [2]. OTC use of Alli should not exceed six months — and if you haven't lost at least 5% of initial body weight within 12 weeks, it's worth speaking to your pharmacist or doctor to review your plan [2] .
