Xenical (Orlistat) 120mg — Weight Loss Capsules pricing
Transparent and competitive pricing plans designed to help you manage your weight loss journey effectively.
What is Xenical, and how does it differ from Alli and Orlos?
Xenical (orlistat 120mg) is a prescription medicine (POM — Prescription Only Medicine) that contains the same active ingredient as Alli and Orlos, but at double the dose.
While OTC orlistat blocks approximately 25% of dietary fat absorption [4], Xenical's 120mg dose blocks approximately 30% [2] — and because it requires a clinician to prescribe it, the full treatment pathway is medically supervised from start to finish.
The key distinctions between Xenical and the OTC alternatives are:
| Distinction feature | Orlistat 120mg | Orlistat 60mg |
| Dose | Xenical 120mg | Alli & Orlos – 60mg |
| Prescription status | POM — you need a clinician to assess and prescribe it | OTC – can purchase without prescription |
| Eligibility threshold | BMI 30+ for Xenical | BMI 28+ for OTC alternatives |
| Fat-blocking efficacy | Approx. 30% at 120mg | Approx. 25% at 60mg |
| Supervision | Xenical is always used under clinical oversight | OTC orlistat can be self-directed |
For patients who have tried OTC orlistat without sufficient results, or whose BMI and health profile point toward a prescription-strength option, Xenical represents a meaningful clinical step up — without the lifestyle changes required to use injectable GLP-1 treatments such as Mounjaro or Wegovy.

Set off on the right foot with weight loss medication that’s proven to help you hit the ground running and reach your goals faster.
GLP-1 medications reduce cravings by directly influencing the brain's reward centre, reducing the urge to reach for unhealthy snacks.
GLP-1 medications help to lower the body's natural weight ‘set point’, so weight can continue to be lost without hitting a plateau.
By improving insulin sensitivity, GLP-1 medications can help manage weight for everyone - even those with slower metabolic rates.

How much weight could you lose?
Clinical trials for prescription weight loss medication show significant, sustained results for eligible adults. On average, patients in published studies lost between 14% and 21% of their body weight over 12 to 18 months — that's the equivalent of 12–18kg for someone who weighs 85kg.
Your individual results will depend on your starting weight, the treatment your clinician recommends, and how closely you follow your programme. Use the calculator below to see an estimate based on your details.

Frequently asked questions
What is the difference between Xenical and orlistat?
Orlistat is the active ingredient — the drug itself. Xenical is a branded product that contains orlistat 120mg. Other products containing orlistat include Alli (60mg OTC) and Orlos (60mg OTC).
Xenical is the only prescription-strength orlistat available in the UK at 120mg. When people refer to "prescription orlistat," they typically mean Xenical. Can I buy Xenical without a prescription?
No. Xenical (orlistat 120mg) is a Prescription Only Medicine (POM) in the UK and cannot legally be dispensed without a valid prescription from a registered clinician.
At Bliss, you can access Xenical online through a fully remote consultation with a GPhC-registered clinician — but this is a genuine clinical assessment, not an automated process. Be cautious of any website claiming to sell Xenical without a prescription. Is Xenical stronger than Alli?
Yes. Xenical contains orlistat 120mg — double the dose of Alli (orlistat 60mg). At 120mg, Xenical blocks approximately 30% of dietary fat absorption compared to approximately 25% with Alli. Clinical trials also show average weight loss of approximately 10.2% at 12 months with Xenical, compared to 6.1% with placebo [1]. Xenical also comes with a higher risk of gastrointestinal side effects if dietary fat intake is not carefully managed.
What is the difference between Xenical and Alli — which is better?
Xenical contains orlistat 120mg (prescription-only, BMI 30+ threshold); Alli contains orlistat 60mg (OTC, BMI 28+ threshold). Xenical produces greater average weight loss but requires clinician oversight and is not available without a prescription.
"Better" depends on your situation: if your BMI is 28–29 and you want to self-manage, Alli is appropriate. If your BMI is 30+ and you want maximum orlistat efficacy with medical support, Xenical is the stronger option. How long does Xenical take to work?
Xenical begins blocking fat from the first capsule. Meaningful weight loss is typically visible after four to eight weeks of consistent use alongside a low-fat diet.
Most patients see the greatest rate of loss in the first six months; beyond that, weight loss may plateau or slow, which is normal. Continuing treatment for up to 12 months under clinical supervision is appropriate for most eligible patients.hat BMI do I need for Xenical?
Xenical is prescribed for adults with a BMI of 30 kg/m² or above. Patients with a BMI between 28 and 29.9 may also be eligible if they have at least one weight-related health condition, such as type 2 diabetes, hypertension, or dyslipidaemia. If your BMI falls below 28, OTC orlistat (Alli or Orlos) is not appropriate either, and our clinicians will discuss the most suitable pathway for you.
Can I take Xenical alongside other weight loss medications?
Xenical is generally not combined with other weight loss medicines without specific clinical reason. It can interact with certain medications including ciclosporin, warfarin, and levothyroxine.
Additionally, caution is required if you are taking amiodarone — orlistat may reduce amiodarone absorption, so your prescriber should monitor amiodarone levels if both medicines are used together.
Always declare all current medications — prescription, OTC, and supplements — in your Bliss consultation. Your clinician will assess interactions and advise accordingly.
